Signed off from care and no aftercare

I had pneumonia at some point but was treated as chest infection by the Birmingham doctors about 3 years ago. I was referred by the GP to the respiratory department with my lungs. My first appointment took place in 2020. I had my follow up at the hospital in 2021. When I saw them, I was signed off from their care with them stating that nothing can be done regarding my condition. Communication was not great from the hospital. No information was provided or direction on what was to happen next. I should have had some aftercare which was never provided.
