A lack of organisation and communication

There seems to be a problem with organisation and communication. The staff, including doctors never seem to know what's going on. Maybe they do not read through medical notes and communication from other providers about their patients. It is very frustrating when you get an appointment and the doctor doesn't know what is going on. You waste most of the appointment telling them things that should already be in their records. It does not surprise me that they have lots of bad reviews. They really need to improve their service.
